Kroger Patient Care
Center
The
Kroger Patient Care Center formed out of a partnership between the Kroger
Company and the Colleges of Pharmacy at the Ohio State University and the University of Toledo. This
partnership serves the community by providing an expanded level of
pharmaceutical care.
The Patient Care Center functions as an
extension of the dispensing pharmacy. Patients have the
opportunity to meet with a pharmacist to discuss their current
medications and disease states, learn about new treatment options and
therapies, and schedule individual consultation appointments for disease
state management.
This
care includes wellness and screening services such as glucose screenings
for diabetes, cholesterol screenings, and bone mineral density screening for
osteoporosis. In addition, disease state management
programs provide pharmacists with the opportunity to work with other
health care professionals to help patients better manage their chronic
diseases. These services are provided by a College Faculty member, a
Kroger pharmacist, a community pharmacy resident and experiential
students. The Patient Care Centers serve as hubs for our Kroger Patient Care System, where many of our pharmacies offer select patient care services to their communities. If you like to teach or have more hands-on experience, become a preceptor at one of 30 college-recognized experiential sites.
Community
Programs
Many of the programs underway at the Patient
Care Center include providing healthy shopping store tours, heart healthy grocery
store tours, health information days for specific
diseases and populations, and medication information at area health
fairs and events.
National
Recognition
Faculty and pharmacists have received
national and regional acclaim for their innovative work with the Columbus Kroger
Patient Care Center. Tara Green, Pharm.D., received the 2005 Merit
Award from the American Pharmacists Association
Academy of Pharmacy Practice and Management, as well as the 2006 Innovative Pharmacy Practice Award from the Ohio Pharmacists Association.
Kristin Casper, Pharm.D., also received the 2004 Innovative Pharmacy Practice
Award from the Ohio Pharmacists Association. Our pharmacy team were key members of the APhA Pilot Implementation and Pilot Provider Advisor Panels for the
PSM:Diabetes project. Kroger and The Ohio State University received the 2007 AACP Crystal Apple Award in recognition of their contributions to learning excellence in experiential education.
